Click here to download and fill out the “New Patient Paperwork” . The forms are also located at the bottom of this post. If you are having difficulty accessing the paperwork, please feel free to come to your appointment 30 minutes early so that you have time to complete it.

At your appointment, we will make copies of your driver’s license and insurance card, if applicable.  There will be additional paperwork to fill out about the office’s privacy and HIPAA policies.  After you have completed the paperwork and the doctor has had a chance to review your paperwork, you will have a discussion with the doctor about the reason for your visit followed by an examination, so that the doctor can thoroughly understand your problem.

Because each patient is a unique person, no examination or treatment is the same.  We perform thorough examinations for several reasons, to make sure we can help you and to make sure chiropractic is safe for you since some serious conditions can mimic muscle or joint pain.

Patients should expect to be given exercises to do at home that will make them feel better and help them to get better more quickly.  The initial appointment typically lasts the longest, taking approximately one hour, while subsequent appointments generally take around 15 minutes.
